1. All forms must be completely filled out according to your membership affiliation

- Membership Application Form
- Electronic Fund Transfer Form (EFT)
- Visitor Card Form
- Attach copy of Government issued ID
- Attach one of the following:
1. Marriage License
2. Civil Union CertificateOr attach three of the following:3.Driver’s License with the same address
4.Joint checking account
5.Proof of Assignment to Beneficiary Rights
6.Proof of joint ownership of a residence or motor vehicle
7.Mutual designation on a will
8.Irrevocable living trust
9.Durable property and health care powers of attorney.

4. You will receive an email confirmation that your application was received and the next step in this process. If you have not
received an email within 2 working days, please contact us directly to ensure we received it.
